Gnalla Community Pty Ltd is currently looking for full-time disability support worker/ Aged or Disabled Carer/Personal Care Assistant with minimum 1-2 years experience. Our business is located at Northern Perth WA. We are looking for compassionate, reliable, and experienced disability support workers for our clients.

You can make a difference in our clients' lives by supporting them with daily living activities and domestic duties and empowering them to achieve their life goals.

Working in a team, you will facilitate the delivery of quality disability services and achieve positive outcomes with service users in a positive and proactive environment.

You will be actively supporting their individual needs and creating a safe environment. The annual salary is between $73,000 - $80,000.Key

**Responsibilities**:
**Support participants in working towards achieving their individual goals.** Assist with daily living activities, including cooking, housekeeping, hygiene, shopping, medication administration, and community access.

**Help participants build their confidence.** Liaise and collaborate with clients, their community, and community & disability support agencies.

**Monitor and review participants' progress through the service and against their support plans.** Utilize positive behavior strategies.

**Support clients in participating in age-appropriate activities and tasks, including employment, in-home disability support, and community access.**

Adhere to the medication administration policy, work health and safety procedures, and client rights policies, including privacy and confidentiality.

**Monitor the safety and well-being of people with disabilities and report any concerns to the Manager.**

Ensure completion of appropriate documentation; relevant information on disability support should be recorded in client notes and communication books as required. **Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.**

Work with clients, family/carers, relevant case managers, and other professionals to support the client's individual goals.
