Southern Cross Care QLD currently have an opportunity for a proactive and self-directed Community Client Facilitator to join our team based in Raceview. In this role you will plan and implement the delivery of the Home Care Package Program (HCP) ensuring quality consumer care outcomes are met as well as providing support to both internal and external stakeholders associated with the Home Care programs in the West Moreton area.

**What are the responsibilities?**

Duties of the role will include:

- Providing high level case management for customers on the Home Care Package Program (HCP) ranging from Levels 1 to 4 and where applicable top up services using block funding or fee for service.
- Undertaking SCCQ Home Care Admission and on-boarding process
- Leading clinical governance and manage the quality management framework within the Community Services to ensure care standards are met and quality issues notified as per escalation guidelines as required
- Coordinating the planning and implementation of a Model of Care which provides consumer centric quality care services in an environment of innovation.
- Facilitating client and network engagement in the customer’s service plan
- Ensuring that the Customer’s physical, social, emotional and clinical needs are met, including liaising with medical professionals as required
- Management of a prescribed caseload including assessment and care planning to remain within budgetary constraints of the service users designated programs
- Advocating internally and externally to ensure systems and services protects the rights of the consumers and give greatest customer control over decision making
- Supervision and management of localised team of personal care staff

**What you will bring to the role?**
- Case Management Society of Australian Registrations (desired)
- Current AHPRA Nursing or Allied Health Registration (desired)
- Current First Aid and CPR (desired)
- Relevant understanding and experience in the delivery of health services and/or community-based services as well as complex case management.
- Demonstrated understanding and knowledge of the needs of the clients from various funding programs (e.g. Home Care Packages and Commonwealth Home Support Programs)
- Demonstrated ability to lead and motivate staff to provide positive client centric outcomes
- Understanding of the Aged Care Sector and Community Program guidelines and service requirements
- Strong conflict resolution and negotiation skills
- Demonstrated awareness and understanding of issues confronting aged consumers in a home or community setting
- Demonstrated knowledge and experience in aged program management (assessment, coordination and budgets) and understanding the care needs of people within the aged sector
- Demonstrated ability to supervise and lead staff is desired
- Demonstrated ability to plan, co-ordinate and organise own work and work of team members
- Exceptional communication skills, both written and verbal and the ability to relate to a variety of people
- Strong knowledge of the relevant Acts and prescribed requirements
- Planning and organisational skills in managing self and external contacts
- Experience in the Microsoft Office suite and exposure and/or ability to acquire skills for an in-house client database system.
- Excellent interpersonal, communication (listening, written and verbal).
