**What You Will Be Doing**

Peninsula Health is seeking a Grade 2 social worker with experience working in hospital settings to join our team. This role will be part of dynamic multidisciplinary teams providing safe, personal, effective and connected care to patients in our Emergency Department and inpatient settings. The team will work proactively and flexibly to ensure patients receive timely care in the right setting and that unnecessary admission to hospital is avoided.
Grade 2 Social Workers at Peninsula Health work are integral members of multi-disciplinary teams, providing quality care to patients and their families across a range of acute wards, including neurological and general medical wards. This is an opportunity to progress your Social Work career, with support given to progress towards the provision of consultation and supervision to Grade 1 Social Workers and Social Work students.

**What You Need**

We anticipate you will have:

- Member of or eligibility for AASW.
- Two or more years post qualifying experience in a hospital or related setting.
- Excellent skills in comprehensive assessment, discharge planning and counselling.
- Demonstrated ability to work collaboratively.
