**The Opportunity**

Our client is seeking an experienced and dedicated Clinical Case Manager or Registered Nurse to join their team in Sydney's Upper North Shore.

Key responsibilities include:

- Support the entry process and smooth transition for residents, coordinating with various stakeholders.
- Oversee Clinical Case Managers' portfolios, providing monthly reports and ensuring proper understanding of roles.
- Identify eligibility for government-funded care and provide training and support to team members.
- Coordinate aged care assessments and manage Home Care Packages (HCPs) through relevant portals.
- Maintain positive relationships with the team, residents, representatives, doctors, and allied health providers.
- Establish a network of health and community services, engaging with the community to promote a positive perception of freedom in the Community.
- Current AHPRA registration as a Registered Nurse (Division 1).
- Experience in the community care sector/home care package management and previous aged care nursing experience.
- Strong interpersonal and communication skills with the ability to work in a multidisciplinary team.
- Effective liaison with key stakeholders, including consumers, families, and management.
- Demonstrated focus on resident goals, independence, and re-ablement.
- Knowledge of relevant equipment, legislation, and policies.
- Understanding of the roles within a multidisciplinary care and service team.
- Adherence to workplace health and safety practices, reporting hazards, and maintaining personal safety.
