**Disability Support Worker based in Chambers Flat & Logan**

Are you compassionate, dedicated, and ready to make a meaningful impact in the lives of others? We are seeking a **Support Worker** to join our team for **day and night shifts**, providing essential care to a clients with **Dementia & Complex Mental Health**. As a key member of our care team, you will provide comfort, assistance, and companionship during the night, ensuring our client's safety, dignity, and well-being are maintained throughout each shift.

**About the role**:

- To support the participants in home and out in the community.
- To know and understand the care and support of the participant
- To undertake the tasks detailed in the participants' Care Plan using a person-centered approach and in the least intrusive way
- To provide personal care and support to participants with a wide range of needs, illnesses and disabilities
- To take responsibility for the safe handling of property and equipment belonging to the participants
- To maintain good communication and develop effective working relationships with participants
- Helping clients with medication reminders, ensuring medication is taken as prescribed
- Offering emotional support, engaging in conversation, and encouraging social activities to combat isolation and promote mental well-being.
- Keeping accurate records of care provided, any changes in the client's health, and reporting incidents or concerns promptly to the team lead.

**Job Details**:

- **Qualification Requirement**:Cert 3 in Disability / Individual Support
- **Minimum 2 years' experience** supporting clients who have Dementia and Complex Mental Health.
- Current medication management certification required. Experience with medication administration and documentation essential.
- **Skills** : Communication, Proactive, Leadership
- **Pay**: Competitive rates based on SCHADS
- **Expected Hours**: 24:7 roster including overnight support.
- Minimum availability 3 shifts per week (24- 36 hours)
- **Required Licenses/Certifications**: Full Driving Licence, Working with Children Check, NDIS Workers Screening, Police Check, First Aid and CPR
- Reliable vehicle with comprehensive insurance for community transport duties
- **Work Authorization**: Australia (required)

**Job Type**: Casual

Pay: $38.15 - $41.76 per hour

Expected hours: 24 - 32 per week

**Education**:

- Certificate I - IV (preferred)

**Experience**:

- Mental Health : 1 year (preferred)

- First Aid Certification (required)
- CPR Certification (required)
