Our Vertical Villages offer lifestyle freedom for our participants, and we’re looking for a Support Coordinator to work with us on the transition period for clients looking to make changes.

**The Position**
The Transition Planner role is designed to serve as a ground-floor contact for our clients and their stakeholders who come to us for help navigating the NDIS and facilitate the transition into supported accommodation.
The position involves dealing with NDIS accommodation funding, understanding housing transition requirements and timelines. In this role you will also be required to maintain high quality case notes and data collection to manage moving parts of the transition, establish relationships and external stakeholder networks to support role delivery. Specifically, this dynamic role will:
- Oversee transitions, making sure all parties are running to the timeline, all tasks are delegated, and issues are worked through
- Define funding/transition timelines and client profile details to assist with timely and smooth transitions
- Liaise with internal departments, co-designing transition plans
- Strengthen existing stakeholder relationships through understanding what we can do to make transitions smoother and more efficient
- Build new stakeholder relationships through analysis of communities, existing programs/supports, current relationships and identified gaps in the system
- Define and refine transition processes, templates and documents, in collaboration with stakeholders and internal teams
- Identify limitations, pressures and concerns internally and externally, working through them in collaboration with stakeholders
- Analyse support needs and identifying limitations, risks, and pressures
- Analyse reports and NDIS documents, utilising your NDIS knowledge to achieve quality person-centred outcomes for our clients
Your ability to manage priorities, document thoroughly, delegate tasks and work to timelines will be crucial for success in this position.
**About You**
To thrive and succeed in our team you will bring:
- Certificate III or IV or above in Community Services, Disability support or other relevant qualification in justice correction or welfare studies.
- Minimum of three years of experience in as a Coordinator of Supports, Local Area Coordinator or a similar role would be beneficial.
- Knowledge and experience in data analysis, NDIS framework and pathways for sustainable outcomes.
- Willingness to work with systems and documentation that are evolving; We are looking for a problem solver who enjoys seeing things through.
- Strong negotiation, conflict resolution, communication and mediation skills.
- Ability to build positive relationships, communicate with and advocate for people of diverse backgrounds and abilities.
- Knowledge and competency in project management.
- Demonstrated capacity to work flexibly, possessing the ability to manage competing demands, and able to effectively set boundaries and limits where required.
- Ability to be adaptable and act appropriately in a fast paced environment.
