**About the Role**

Join our team as a Social Worker, Grade 2 in our healthy Mothers Health Babies Program, based in Springvale. This is a parental leave position and is 0.6 EFT, 3 days per week.

In this role, the Social Worker provides Outreach support to pregnant women experiencing psychosocial complexity to help improve the health and wellbeing outcomes for themselves, their babies and families. Support continues until approximately 6-8 weeks post birth once women are linked with Maternal and Child Health and other appropriate supports.

Key responsibilities will include:

- Improve women's access and attendance at antenatal and postnatal services through assertive outreach, referral and supported linkage to services.
- Improve women's access to a range of support services which may include health, mental health, drug and alcohol, family violence, welfare, legal, multicultural, housing, material aid, financial or education services.
- Deliver health promotion messages that aim to reduce risk behaviours and promote healthy behaviours.
- Advocate and work collaboratively with relevant agencies and programs to deliver appropriate services to the women.
- Provide support, education and information around pregnancy, birth, preparation for baby and early parenting.
- Provide support to other members of the family and referrals to other services to ensure the supports are available following birth.

The Child and Family Program including Healthy Mothers Health Babies (HMHB) is compromised of numerious disciplines including Social Workers, Community Development Workers and a Nurse/Midwife that that are dedicated to improving the health and wellbeing outcomes for pregnant women, their babies and families.

The Healthy Mothers, Healthy Babies Program aims to reduce the burden of chronic disease and reduce health inequity by addressing maternal risk behaviours and providing support during pregnancy. The program will be targeted in the Cities of Casey and Greater Dandenong that have high numbers of births, higher rates of relative socio-economic disadvantage and lower service availability. The program is committed to providing women with high quality care, support, education and advocacy throughout their pregnancy and following birth, which embrace family centred, strengths based and evidence-based practice, within a Social Model of Health.

**About You**

You are a dedicated and compassionate social worker with a strong understanding of the complex systems that impact children and families, particularly those who are vulnerable or at risk. With demonstrated experience supporting clients across diverse service areas—including Family Violence, Child Protection, Mental Health, Drug and Alcohol, Sexual Assault, and the NDIS—you navigate these frameworks with confidence and professionalism.

With well-developed communication skills, both written and verbal, you build strong relationships and convey sensitive information with clarity and respect. You thrive both independently and in team environments, consistently demonstrating motivation, adaptability, and a commitment to ongoing learning. Above all, you are guided by a deep awareness of the broader social, legal, and cultural contexts that shape the lives of the individuals and families you support.

Education and registrations/licences required for this role are:

- Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
- Written evidence of eligibility for AASW membership for overseas qualifications is required.
- Accredited Social Worker: As approved by the AASW is advantageous.
