_Health and Leisure to You_ provides social and community care services to people with Younger Onset Dementia and related cognitive conditions. We are looking for an experience and qualified Support Worker to provide direct care and support services to Participants and their carers. This is predominately from our Day Centre in Richmond NSW but extends to community outings and limited in-home support around Western Sydney.

Duties include:
**Personal Care**: In line with the Participants care plan, provide personal care assistance with bathing/showering, hygiene, dressing, grooming, and other personal care tasks relating to hygiene and diet.

**Social support**: Assist Participants to remain active in their community and personal relationships, as well as providing a space for them to make new connections.

**Transport**: Provide transport support to enable service users to access community and social activities and/or medical and specialist appointments.

**Medication monitoring**: If required, and in accordance with instructions in the Care Plan, monitor Participants taking their medication.

**Social Groups**: Assist with planning, implementing, and evaluating a program of Leisure and Lifestyle activities which is designed to meet the individual needs of each Participants. Encourage and support all Participants to participate in a wide range of interests and activities. Implement client skill and activities programs either individually or as part of a team.

You must have:
**Certifications**:

- Certificate III or IV in:

- Leisure and Lifestyle;
- Aged Care;
- Disability.
- First-Aid and CPR certificate.
- NDIS Worker Orientation Module.

**Transport**
- NSW Drivers License
- Suitable vehicle
- Current registration and comprehensive insurance.

**Background Checks**
- Ability to receive an NDIS Worker Screening Check within the first month of employment.

**Salary**: $30.00 - $40.00 per hour

Expected hours: 20 - 25 per week

Schedule:

- 8 hour shift
- Afternoon shift
- Day shift
- Morning shift
- Rotating roster
