We’re on the lookout for a full-time **Health Linker** to join our mission. As a Health Linker, you’ll collaborate with clients, allied health providers, general practitioners, and community services to streamline access and coordination of essential support that meets the unique needs of each individual.

**What You’ll do**:

- Deliver holistic, coordinated support to clients engaged in the WARATAH Program, implementing care plans and connecting them to vital health and social services.
- Collaborate with clients to craft personalised support plans that enhance health and well-being, re-establishing connections to community groups and essential services.
- Help individuals tackle broader challenges affecting their health, such as financial difficulties, housing issues, unemployment, isolation, and caregiving responsibilities.
- Provide tailored guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices

**What You’ll need**:

- **Relevant tertiary qualifications** in Nursing, Aboriginal Health, Allied Health, Community/Human Services, Social Work/Social Science, or equivalent.
- A solid understanding of the** broader determinants of health**—including social, economic, and environmental factors—and their impact on communities.
- Proven **experience in co-developing person-centred support plans** that prioritise client well-being and aspirations.
- **Advocacy skills** to enhance client access to programs and inspire community participation.
- **Exceptional communication and interpersonal abilities**, particularly with culturally and linguistically diverse (CALD) backgrounds and First Nations communities.
- **Experience in partnership building**and collaborative work across various organizations to achieve client-focused outcomes
