**Position Overview**
As a Support Coordinator at Mars Hill Care, you will play a crucial role in ensuring that our clients receive the highest quality of care and support. You will be responsible for coordinating services, managing client relationships, and collaborating with various stakeholders to create individualised care plans. This role requires excellent communication, organisational, and problem-solving skills.
**Key Responsibilities**
- Develop and implement individualised care plans in collaboration with clients, families, and other care providers.
- Coordinate and manage services to ensure seamless delivery of support to clients.
- Monitor and review the effectiveness of care plans, making adjustments as necessary.
- Advocate for clients to ensure their needs and preferences are met.
- Maintain accurate and up-to-date records and documentation.
- Foster strong relationships with clients, families, and other stakeholders.
- Ensure compliance with all relevant regulations and standards.
- Provide ongoing support and guidance to clients and their families.
**Qualifications**
- Bachelor’s degree in Human Services, or a related field.
- Minimum of 2 years of experience in a support coordination or case management role.
- Strong understanding of person-centred planning and care coordination.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organisational and time-management skills.
- Proficiency in using case management software and other relevant tools.
- Valid driver’s licence and reliable transportation.

Pay: $73,902.76 - $87,172.35 per year
