Your Side is a one-stop shop to arrange the care and support that allows people and families to get on with living a good life.

The care finder program delivers a national, face-to-face network of care finders. It provides support for vulnerable older people to interact with My Aged Care, access aged care services, and access other relevant supports in the community. The program aims to target harder-to-access groups in the community who are unlikely to engage with the system due to barriers such as language, cognitive decline, or a reluctance to deal with the government systems.

**_The Role:_**

Work for a reputable NFP in the Care Finder Program to perform a blend of case management and community outreach.
- Support a caseload of people to engage with the aged care system, with key target communities including Aboriginal and Torres Strait Islanders, CALD, LGBTQIA+, and people who are likely to disengage with Aged Care due to past trauma. This will likely include people who have been homeless, experienced DV, or have ongoing mental health or AOD concerns.
- Build trust with vulnerable clients using trauma-informed and client-centered practice.
- Attend assessments and appointments with the client as well as support them to build the skills to achieve their goals, particularly those relating to accessing aged care support.
- Keep records that mean that program reporting requirements are met.
- Engage in one day a week of community outreach to generate referrals in key target groups.
- Attend events and meetings such as interagencies to engage with services and community groups.

**_About yourself:_**
- You identify as Aboriginal and Torres Strait Islander.
- You have 2-3 years of experience as a caseworker or social worker.
- You have a background in working with Aboriginal and Torres Strait Islanders.
- You have qualifications in Psychology, Social Work, or another relevant field.
- You have a person-centered approach and are trauma-informed.

**_Desirable but not required:_**
- You have knowledge of the aged care space.

**_Requirements for the role:_**
- Double vaccinated against COVID-19 OR valid medical exemption
- Able to conduct Police Check
