**_About the role_**;
We are seeking enthusiastic, passionate, and caring individuals to join our team supporting people with disabilities. Your work will support people achieve their individual lifestyle goals, and as we support people of all ages and interests, each workday will be different and innovative.
- **The role may involve**_;
- Provide 1:1 direct care assistance to each client in accordance with their care plan and as directed.
- Monitor the safety and well-being of the clients and report any concerns to supervisor.
- Support clients to access activities and outings in the community.
- Manage accurate and detailed shift reports, incident reports and progress notes.
- Work with client, family/carer, relevant case manager and other professional to support the client’s individual goals.
- Actively contribute to a team environment through open communication and participation in regular staff meetings.
- Participate in performance development meeting to identifying ongoing training and support opportunities.

**_Essential Criteria_**;
- NDIS Workers Screening Check Clearance
- Satisfactory National Police Check for 'Employment in Disability' (issued less than 1 year ago)
- Satisfactory Working with Children's Check (Employee)
- Current First Aid & CPR Certificate
- Current Driver’s License & your own car
- A Certificate III or IV in one of the following or equivalent: Individual Support/ Home & Community Care/ Aged Care/ Disability/ Nursing
- Must be able to provide Covid Vaccination certificate including booster
