**Our Journey**

The PERFORM [Personalised Exercise Rehabilitation FOR people with Multimorbidity] trial will test whether a program of rehabilitation designed specifically for people with multimorbidity can improve patient and health service outcomes compared to usual care or standard cardiac and pulmonary rehabilitation. Monash Health is one of the nine sites for the PERFORM trial in Australia, funded by the Medical Research Future Fund.

**About the Role**

The AHA for the PERFORM trial will work under the direction of physiotherapist/exercise physiologist in assisting to recruit participants, supervising delivering the PERFORM intervention, monitor and report changes in the participants' health status to the treating physiotherapist/exercise physiologist, document treatment in the PERFORM records as directed by the physiotherapist/exercise physiologist in accordance with the trial requirements. Assist in the coordination and organisation of intervention as prescribed by the health professional, maintain accurate participants' records, record the sessions, undertake reception or administrative duties as required.

**About You**

**Must possess one of the following qualifications (essential)**:

- Certificate III or higher qualification as an Allied Health Assistant, Registered Nurse, Certified Fitness Instructor or Personal Care
- Diploma, Associate Diploma or higher qualification in a relevant health science area as determined by the Manager
- Allied Health undergraduate student in relevant discipline having completed a minimum of one year of study
- Certificate courses in a relevant health science area as determined by the Manager
- Eligible for registration as a Registered Nurse Division 2
- Overseas Allied Health Qualifications - minimum qualification is completed diploma in a recognised discipline
- Previous experience within a hospital setting or working with a multidisciplinary health team
- Clinical experience in Rehabilitation and Aged Care
- High level of English literacy
- Strong verbal and written communication skills
- Sound knowledge base of medical terminology

**_Desirable:_**
- Current First Aid and CPR certificate
- Advanced computer skills
- Previous Supervisory responsibility of an area and/or staff members
- Experience assisting Allied Health Professionals
