**About the Role**

The Team Leader is a clinical leadership role responsible for supporting clinical outcomes and the management of staff as directed by the Intensive Support Manager. The role has a clinical component to deliver client care as needed. The service provides care to people with chronic conditions and/or complex biopsychosocial issues and manages clinical. The incumbent will work closely with the Intensive Support Program Manager and other Team Leaders to ensure consistent procedures and practices are utilised to manage the day-to-day operational tasks. Coordination of department activities including but not limited to:

- UKG, rosters
- Coordinating case reviews
- Referrals and waitlists. monitoring
- Supporting gives me 5 and regular monthly catch up with employees
- Contribute to innovation within and further development of the program
- Assist and deputise for the Program Manager as directed
