An exciting opportunity exists for a motivated and enthusiastic Social Worker to bring their existing skills as a health Social Woker to the Transitional Support Unit, (Social Worker Gr2).
As part of the Department of Health's Workforce Development Plan this position sits within the **Transition to Mental Health Practice** program.
This is a full-time fixed term position 12months. The TSU SW role operates Monday - Friday 8:30am - 5:00pm.
The TSU provides a step up - step down facility for people experiencing mental illness and cognitive or intellectual impairment. TSU delivers rehabilitation and recovery care to clients with both learning difficulties and mental illness. Specifically, the target group are those who require medium/long-term residential care and would benefit from a therapeutic support and recovery focused model of care but do not require an intensive clinical environment.
TSU aims to provide integrative and holistic care by offering a range of services on-site and progressively off-site through engagement with disability specific mental health and other mainstream services, and with the community. Coordination between the mental health, health and disability sectors is central to the model of care.
TSU promotes collaborative relationships with external providers, including Victorian Dual Disability Service (DDS), Austin Health’s Brain Disorders Program, state-wide mental health services, specialist health services, disability service providers (NDIS) and other Non-Government Organisations.
The primary aim of this Social Worker on TSU is to work with external services to provide targeted plans to facilitate discharge for those residents who are ready to move from the TSU.
This is a great opportunity, for a dedicated Social Worker to have a big impact on the lives of residents within the TSU.
As part of the **Transition to Mental Health Practice** program, this position will be supported by a SW Clinical Educator and provided with specific training and guidance around mental health diagnosis, treatment and intervention.
Other supports offered via this program include dedicated professional development time, and dedicated peer support and connection with other SWs as part of the Transition to Mental Health Practice program, locally and across the state.
