**What You Will Be Doing**

The Aged Persons Community Mental Health Team (APCMHT) is a specialist multi-disciplinary and integrated service that provides a broad range of interventions, care coordination and support for persons 65 years and older experiencing severe and persistent mental health disorders and their carers.

The Aged Community Mental Health Clinician will plan, evaluate and review care, undertaking face to face assessments and contributing to recovery care plans. The role requires frequent liaison with external services and the internal multi-disciplinary team.

This position is open to RN’s, Social Workers & Occupational Therapists with Mental Health experience. We have a Temporary PartTime position available - 32 hours a week.

**What You Need**
- Registration as a Registered Nurse, with Post Graduate in Mental Health qualification, or a Social Worker or Occupational Therapist qualification and Registration
- Proven ability in the management of clients with a major mental illness and in the techniques of assessment and crisis intervention, and recovery frameworks
- Experience working in an acute inpatient or community mental health setting
- Current Victorian Driver’s License
