**About the role**

Baptistcare Durham Green and Blue Hills are seeking an experienced Lifestyle Coordinator to oversee the Lifestyle and Activities calendar and lifestyle care planning. The role involves developing, implementing and monitoring a Lifestyle program that optimizes the quality of life for our residents.

You will coordinate and manage events and group activities and lifestyle resources, build and develop volunteers, train and support staff in activities to ensure provision of optimum level of person-centered service to all clients.

**This position is permanent part time, the hours are 71hrs per FN. **3 days at Menangle one week and 2 days at Prestons the other week**

Skills and experience:

- Diploma or Certificate IV in Lifestyle and Health (or related) **Essential**:

- Current unrestricted driver's license
- Previous experience working in an aged care setting (Essential)
- Proven experience developing and coordinating lifestyle plans particularly for residents living with dementia
- Current First Aid Certificate
- Knowledge of the Aged Care Accreditation Standards and related outcomes
- Strong interpersonal skills and computer literacy

Located in the heart of Menangle’s charming town centre, Durham Green Centre aged care home caters to residents who need some additional support with everyday tasks, while maintaining some independence.

The team at Durham Green Centre strive to empower residents with the support they need, providing care and assistance 24 hours a day.

Our caring team are here to make the transition into aged care as easy as possible and cater for the individual needs of each resident. The team’s round-the-clock care program provides assistance with personal hygiene, mobility, nutrition and hydration, incontinence, medication, and transfers featuring wheelchairs or walking aids. We also offer individual support and treatment for recurrent confusion including cognitive impairments and behavioural disorders.

**About BaptistCare Blue Hills**

Located in the heart of Prestons, BaptistCare Blue Hills Manor is a welcoming and homely aged care residence, offering 67 single bedrooms. located onsite with BaptistCare Blue Hills Retirement Village.

Co-located with BaptistCare Blue Hills Retirement Village, the home offers 24-hour, care and support to residents. Our support caters to all needs and requirements, whilst empowering residents to retain their sense of independence.

There are plenty of areas for residents to enjoy, from the communal lounge room to beautifully landscaped gardens, our home is your home to enjoy.
