Are you a passionate and experienced Registered Nurse looking for an opportunity to make a real impact on the lives of older Queenslanders? We are seeking a dedicated Care Manager Community Nurse to coordinate and enhance our aged care services in the Southeast Queensland region.

**Your Role**:
As Care Manager Community Nurse, you will be a key member of our team, responsible for coordinating and delivering high-quality, evidence-based care to clients in their own homes. Our programs are designed to support early hospital discharges and prevent unnecessary admissions by providing comprehensive in-home care.
**Key responsibilities include**:
- **Clinical Leadership**: Deliver high-level clinical care, including wound care, fall prevention, and risk assessments.
- **Case Management**: Oversee initial and ongoing client case management, ensuring care plans are adjusted to meet clients’ evolving needs.
- **Team Collaboration**: Work closely with medical professionals and allied health services, including Nurse Navigator programs, as well as other QAADS team members.
- **Client-Centred Care**: Monitor, implement, and modify care plans in collaboration with clients and families, empowering them to achieve personal and clinical goals.
- **Policy Development**: Support the development and improvement of policies and procedures for care workers to ensure best practices and compliance with industry standards.
**What We’re Looking For**:
**Essential**:
- Current AHPRA registration as a Registered Nurse.
- Experience in multidisciplinary teams within a healthcare setting.
- Proven clinical expertise in hospital discharge planning, case management, and client risk assessments.
- Strong problem-solving, clinical reasoning, and communication skills.
- A client-focused approach with an ability to manage priorities in a fast-paced environment.
**Desirable**:
- Two years of experience with aged care and home services.
- Knowledge of relevant industry regulations and quality standards.
