**About the role**:
The overall purpose of the
**Practitioner - Out of Home Care** is to provide case management to children or young people within residential care services or programs.

The role will assist to develop participation in positive age-appropriate activities; general physical and emotional wellbeing; engagement in schooling, employment or other training; relevant life skills; and positive connection with family and other significant people.

We are recruiting for a full-time fixed term contract for three months with possibility of extension for an experienced Case Manager/Practitioner to be based at our Maroochydore service managing caseload from our Sushine Coast Residential Services.

**About You**:
To be successful in this role, you will have the following experience and capabilities within a similar role:

- Applied knowledge and understanding of youth justice and child protection frameworks including the Youth Justice Act and Child Protection Act (1999) and other relevant legislation
- Understanding of child and adolescent development including impacts of trauma, attachment disorder and grief & loss
- Assessment of complex to extreme needs of young people and the implementation of strength based care plans that target areas for development
- Ability to respond appropriately in crisis situations; highly developed problem solving, decision making and negotiation skills
- Able to work effectively in a team and independently; collaboration with stakeholders and meeting service delivery demands
- Strong verbal and written communication skills; report writing and meeting planning

**Additionally you must possess**:

- Current Blue Card
- Current Licenced Care Service (LCS)
- Current Queensland Driver Licence
- **Degree in Health, Human Services, Behavioural Science, Social Work, Psychology **_(this is an essential requirement of the role)_
- Ability and willingness to travel within the service region
- Ability and willingness to work outside of standard business hours and provide on-call support as required
