Are you an experienced Disability Support Worker with the skills and patience to support individuals with complex needs? Diligent Care is looking for a dedicated professional to provide one-on-one support for a young adult male participant with ASD-3 and Global Developmental Delay (GDD). This participant is **non-verbal** and exhibits **physical behaviours of concern**, including aggression towards self and others.
**About the Role**
This permanent position involves supporting the participant at our **day program in Kenthurst, NSW (9:30 AM - 3:30 PM, 6 days a week)**, with potential future transition into **Supported Independent Living (SIL) accommodation**.
**Key Responsibilities**:
- Providing **personal care, toileting, transport, and companionship**.
- Supporting the participant in **daily activities and community outings**.
- Following and implementing the **Behaviour Support Plan** effectively.
- Documenting incidents, progress, and daily support activities accurately.
- Ensuring safety by using appropriate **restrictive practices** where required.
**What We’re Looking For**:
- **Experience with physical behaviours of concern**, including aggression.
- Understanding of **restrictive practices**:
- **Proficiency in English** for documentation and communication.
- Ability to follow structured support plans and work in a team.
- **Preferably a physically built, taller male** to match the participant’s support needs.

Diligent Care is seeking dedicated and compassionate people to provide one-on-one support for participants with **limited mobility and mental health conditions**. This role involves assisting participants in their **own home** and within the local community, ensuring they receive the support needed to live a fulfilling and independent life.
**About the Role** This position involves working closely with the **participant in their home environment**, providing personal care, assistance with daily activities, and social engagement. The role requires a commitment to building **trust**and maintaining a stable and **supportive routine**.
**Key Responsibilities**:
- Assisting with **personal care, toileting,**and **hygiene**.
- Supporting the participant in **daily living tasks, including meal preparation and household duties.**:
- Facilitating community access and engagement in social activities.
- Following and implementing individualized care plans effectively.
- Documenting daily support activities and any incidents accurately.
- Ensuring safety and well-being through appropriate risk management strategies.
**What We’re Looking For**:
- Experience in **in-home disability** or **aged care support**.
- Ability to follow structured care plans and work independently.
- Strong communication and documentation skills.
- A **caring and patient nature**, with the ability to build rapport and trust.
- **Physical capability to assist with manual handling if required.**:
- **Flexibility to work various shifts**, including weekends, early mornings, overnight and evenings.
**Essential Documentation/Criteria**:
- Driver Licence (Red P & above/ or International)
- Registered, insured car
- Valid Australian work authorisation OR A valid visa to work in Australia
- Police Check clearance
- At least 2 COVID-19 Vaccinations
- NDIS Worker Check
- NSW Working with Children Check
- Current and valid First Aid and CPR certificate
