**About the Role**
**Key Responsibilities**
- Conduct AN-ACC Assessments in collaboration with care staff and leadership.
- Design and implement best-practice care plans that reflect individual needs and values.
- Undertake Resident of the Day (ROD) reviews alongside the Facility Manager.
- Ensure assessments are completed within 72 hours of admission and updated as required.
- Consult with residents, families, Div 1 nurses, and external providers to guide care planning.
- Lead continuous improvement initiatives through audits and feedback loops.
- Support carers and deliver emotional, social and clinical support to residents.
- Manage specialised care areas including mobility, nutrition, wound care, pressure care, continence etc.
- Maintain thorough documentation and communication across care teams and families.
- Complete HELF Checks and coordinate required training with PAL and P&C for new staff.
**Essential skills and attributes (selection criteria)**:
- Registered Division 1 Nurse with current National Practicing Registration
- Excellent problem-solving and care planning capabilities
- Strong working knowledge of the needs of the elderly person residing within a residential care facility.
- Exceptional problem solving, assessment and care planning skills
- Excellent AN-ACC operational knowledge
- Self-motivated, proficient with computerised documentation systems and records
- Strong interpersonal skills with the ability to build and maintain working relationships within a multi-disciplinary team
