Perth Disability Services specialises in Disability Care. We are a Registered NDIS Provider delivering Disability Supports to NDIS Participants. This can range from high care, including private in-home care through to social support, daily living assistance, community access, SIL, MTA and Respite services.

**About the Role**

We are looking for Full-Time and Part-Time Passionate Team Members to expand our new division of Support Coordination. As a Support Coordinator, your role will be all about helping NDIS participants achieve the goals set out in their NDIS plans. You’ll work closely with participants to empower them to direct their Lives. This involves assisting them in building a strong network of formal and informal supports and guiding them through complex service delivery systems. You will ensure that individual’s plan is delivered within their plan budget, supporting them to use their budget flexibly to work towards their NDIS Plan goals.

For Part-Time position weekly hours will be roughly around 22-28 hours(negotiable) and salary will be on pro-rata basis.
- **In order to succeed in this role, you will require the following**:_
- Previous experience as an NDIS Support Coordinator
- Diploma of Community Services or other relevant qualification
- Demonstrated understanding of the needs and issues of people living with disabilities and their carers
- Demonstrated experience in needs assessment and management of NDIS participants
- Understanding of NDIS funding instruments with experience in managing client budgets
- Excellent written and verbal communication skills
- Computer literacy
- Valid Clearance Checks
- Valid Driver License

Pay: $80,000.00 - $85,000.00 per year
