Monash Health is excited to announce an opportunity for a Community Development worker to join our FaPMI (Families where a Parent has a Mental Illness) program.
The FaPMI program aims to promote the wellbeing of consumers who are parents, their children, and their families through timely coordinated preventative and supportive action within area mental health services.
As an expansion of our existing FaPMI program, the Community Worker for Children and young People will work closely with our team of FaPMI Coordinators to strengthen the support networks, information, and financial assistance available for young carers across the Clayton/Middle South and Dandenong areas.

**About The Role**
In collaboration with relevant service providers, identifying support needs and options for children, young people and young carers to:
- Strengthen their existing support network within their family context, where safe and appropriate to do so
- Facilitate access to more formal support services where there are identified gaps in informal supports
- Facilitating access to support, information, and connection to children and young people, including referral to appropriate peer support programs
- Supporting children, young people and their families to access financial assistance in liaison with the FaPMI coordinators, through the FaPMI brokerage fund
- Promoting awareness of young carer supports to service providers, including through developing relationships with local schools and other youth focused organisations
- Developing pathways to supports for children and young people
- Supporting delivery of the local AMHWS FaPMI action plan with the FaPMI coordinators
- Participating in FaPMI Statewide activities including working groups and FaPMI CYP Worker community of practice
- Co-facilitation of peer groups for children and young people
- Facilitating access to transport for young people, where appropriate
**About You**
**You will have**:
- A Certificate IV in Community Development or equivalent
- A passion for supporting children, young people (up to 25 years) and their families
- Well-developed interpersonal skills and clear communication skills (verbal and written)
- A strong interest in working in public health
- The ability to work within a multidisciplinary team, contribute to team discussion, and a willingness to take on advice and direction
- Motivation and self-direction
- Demonstrated capacity to be flexible and respond positively to change.

**What you need**:
- A Certificate IV in Community Development or equivalent
- As we support a culture of safety through employee immunisation, there must be documentation provided confirming completed immunisation on employment at Monash Health
- Current and satisfactory Police Check (must also comply with Aged Care Act 1997 Accountability Amendment Principles 2012 if working in Aged Care setting)
- Current and satisfactory Working with Children Check.
- Applicants who are new to Monash Health will be required to enter into an Employment Agreement before commencement
- Applicants who are new to Monash Health must provide evidence of immunisation before they can receive an offer of employment
