**Key Responsibilities**:

- Provide direct assistance to individuals with disabilities in home and community settings.
- Aid with daily activities including personal hygiene, meal preparation, and medication management.
- Develop and implement personalized care plans in collaboration with clients, their families, and healthcare professionals.
- Support clients in accessing community resources, participating in social activities, and pursuing employment opportunities.
- Monitor client progress, document observations, and report any changes in health or behaviour.
- Advocate for clients’ rights and ensure their needs and preferences are considered in care planning.
- Ensure a safe and supportive environment by adhering to health and safety regulations and best practices.
- Participate in ongoing training and professional development to stay current with industry standards.

**Qualifications**:

- Certifications or training in disability support or a related field preferred.
- Previous experience working with individuals with disabilities is highly desirable.
- Strong interpersonal and communication skills with the ability to build trust and rapport with clients and their families.
- Compassionate, patient, and respectful attitude.
- Ability to work independently and as part of a team.
- Valid driver’s license and reliable transportation are required.
- Background check and clearance may be required.
