As a deeply grounded NDIS and Aged Care provider, we require Support Workers on a casual premise with a view towards permanency. One should be adaptable and flexible to work with individuals with diverse special needs and to help frail, older people, including the prematurely aged, living in the community to maximize their independence.

**About the Role**

**Deivian Care is looking for a Support Worker in Mount Dandenong (VIC) and surrounding suburbs **with expertise in case management, social work, and outreach. You will play an important role for many participants who may be excluded from mainstream healthcare, social support, and opportunities, negatively impacting their functioning and independence in the community.

Availability: 5 days (Day availability)

Transportation - Must have Drivers' Licence and car

Suburb - Mount Dandenong and surrounding suburbs or available to work as a local.

**About You**
- A person-centered, values-based approach to service delivery that is informed by a proven commitment to social justice concerns.
- Knowledge and understanding of trauma informed, strengths-based and person-centered care.
- Demonstrated communication and relationship management abilities, as well as the ability to maintain collaborative working relationships with a wide range of stakeholders.

**Job Role and Responsibilities**
- Must have experience in PEG feeding and hoist transfer (Manual Handling)
- Assist participants with Personal Care / Domestic Assistance - Community Access, Showering, Toileting,

**Experience/Certification Required**
- Cert III relevant qualification in Disability/Aged Care/Individual Support/Social Work
- Full VIC Driver's License and your own vehicle
- National Criminal Check
- Working with Children Check
- First Aid & CPR (or willingness to get one)
- Covid-19 Vaccination Certificate
- Manual Handling
