**About the role**
- Assess, support and monitor the mental health of participants using a trauma-informed, recovery orientated approach - this will include completing comprehensive assessments
- Promote and support the physical health of participants by undertaking health screenings and supporting participants to follow up with essential health appointments
- Promote and support the recovery of participants experiencing alcohol and other drug (AOD) challenges by undertaking appropriate screenings and supporting them to access suitable AOD services, ongoing treatment and support options
- Coordinate care and support participants to attend a appointments with a wide range of health professionals including GP’s, Hospital and Health Services, Psychiatrists and Psychologists
- Actively seek referral pathways and provide advocacy for participants to access essential services
- Liaise and build strong connections with a wide range of external stakeholders including Psychiatrists, Psychologists, GP’s and Hospital and Health Services in order to improve access for participants
- Assist with the development and implementation of shared support plans with the wider multi-disciplinary team (Social Workers, Peer Workers, Psychologists and Mental Health Practitioners)
- Participate in case discussions, consultation with team members and clinical review regarding participants’ ongoing support needs
**Role criteria**
- Must have one of the following: AASW eligibility as a Social Worker OR General/Provisional AHPRA registration as a Psychologist OR General AHPRA registration as an Occupational Therapist or Mental Health Nurse OR extensive experience
- Experience in providing coordination of supports for people living with severe and complex mental health concerns
- A comprehensive understanding of the correlation between mental health, physical health, and alcohol and other drug (AOD) use, with a demonstrated ability to assess needs and navigate appropriate support services across these areas
- Demonstrated knowledge and experience in supporting participants through the main duties and responsibilities outlined in the position description (must speak to this in cover letter)
- A well-articulated framework for practice that utilises a trauma-informed and recovery orientated approach
- Exceptional interpersonal and communication skills
- Demonstrated knowledge and experience in computer skills, particularly data management systems, word processing skills and Microsoft Outlook management
- Demonstrated ability to contribute as part of a team to provide a safe, equitable and rewarding workplace
**Additional requirements**
- NDIS Screening Clearance (or ability to obtain one)
- National criminal Record Check - disclosable outcomes considered

Funded by Brisbane North PHN, the centres provide free, integrated support over extended hours for individuals with moderate to severe mental health concerns and complex life circumstances, as well as support for carers and families.
