Support Worker (Support and Outreach Coordinator)

**Preferred Availability**: Mornings, 5 hours per week (set schedule to be determined with the participant).

**About the Role**

We’re looking for a compassionate and reliable support worker to assist with personal care and community access in Singleton, NSW. This role is all about building trust, providing consistent support, and helping the participant maintain independence while engaging in meaningful activities in their community.

**Key Responsibilities**

**Support Duties**:

- Provide gentle and respectful prompting for personal care tasks, such as hygiene and dressing, while maintaining the participant’s dignity and comfort.
- Facilitate safe and enjoyable community outings, encouraging engagement in social and recreational activities.
- Build a positive and trusting relationship through consistent, empathetic care.
- Follow individual care plans to ensure safety and align with the participant’s specific needs.
- A caring and patient individual who aligns with the participant’s preferences, including shared cultural or religious values, if applicable.

**Outreach Duties**:

- Work with local community networks in Singleton to raise awareness about support services.
- Collaborate with our team to connect participants to new opportunities and activities in their area.

**What We’re Looking For**
- A caring and patient individual who aligns with the participant’s preferences.
- Relevant certifications, including:

- NDIS Worker Screening Clearance (required)
- NDIS Worker Orientation Module (required)
- First Aid and CPR Certification (preferred)
- Police Check (preferred)
- A valid driver’s licence and access to a reliable, insured vehicle for transport.
- Strong communication skills and the ability to adapt to the participant’s needs.
- Previous experience in personal care and community engagement is highly regarded.
