Family Preservation and Reunification Practitioner
2 weeks ago
**About the role**:
The Victorian Family Preservation and Reunification (FPR) Response Program, funded by the Department of Families, Fairness and Housing (DFFH), is an innovative initiative within the Southern Melbourne Region, part of the dynamic Family & Community Services Team at our Dandenong office.
This groundbreaking FPR program kicks off with an intensive intervention phase, delivered through a mobile and integrated approach, followed by a sustained service support phase. Our mission is to prevent at-risk children from entering or re-entering care, or to reunify children with their parents. By aligning and enhancing existing child and family services, we provide a seamless continuum of care across the state. We continuously evaluate and develop the program to build a robust evidence base for Victorian child and family services, including Child Protection.
Our integrated and evidence-informed model supports and problem-solves with families, keeping children and young people out of out-of-home care. Join a cohesive team that operates with passion and reports directly to the FPR Practice Team Leader. Be part of an exciting journey making a real difference in the lives of families across Victoria.
