**The Role**:
Due to growth, we are looking for a Permanent Part-time or **Full-time Enrolled Nurse/Home Care Service Advisor **reporting to the Home Care Regional Manager to support the coordination and management of care to our clients who are on Home Care Packages.

**Home base: Banora Point - providing services to clients between Tweed Heads, Banora Point and Ballina.**

As a Home Care Service Advisor you will:

- Complete holistic assessments
- Develop appropriate care plans
- Manage client’s care needs within Home Care Package funding
- Manage referrals to other services when required
- Manage and provide service delivery to a portfolio of home care clients and their families.
- Cover leave for other Registered Nurses and Service Advisors clinical care and assessments
- ** No on-call or after hours**:

- ** Phone and laptop, use of pool car or vehicle allowance by negotiation.**:

- ** Remuneration $35.47 to 37.46 per hour dependent on qualifications**:

**About You**

To be successful in this role, you will have:

- Qualified Enrolled Nurse or Registered Nurse
- Cert 4 in Aged Care
- Excellent communication and interpersonal skills
- High level of organisational and time management skills
- Intermediate-Advance Microsoft Office skills
- Experience in home care, with a qualification in Community Services, Aged Care, Community or Case Management (highly desirable)
- Driver's license and car with comprehensive insurance (required)
- Evidence of COVID-19 vaccinations
