About the Role

We are seeking a skilled and experienced Grade 2 Social Worker to join our Acute Team based at Malvern and Brighton. This role offers the flexibility of part-time or full-time hours and involves delivering psychosocial care and facilitating discharge planning for inpatients.

There is scope to work across palliative and rehabilitation services as needed.

Key Responsibilities- Deliver high-quality, patient-focused psychosocial care- Build collaborative working relationships with patients, families, and hospital staff- Use strong conflict resolution and negotiation skills to support patient outcomes- Manage a complex caseload efficiently and coordinate timely hospital discharges- Supervise and support Grade 1 Social Workers, students, and Allied Health Assistants- Engage in professional development and lead innovative service improvement initiatives

About You

You are a motivated and compassionate social worker who thrives in a multidisciplinary healthcare environment.

You bring:
- A degree in Social Work and eligibility for AASW membership- Minimum 3 years' clinical experience in a healthcare setting- Sound knowledge of social work theory, ethics, and evidence-based practice- Excellent communication, time management, and conflict resolution skills- A flexible, proactive, and solutions-focused approach to your work- Experience in hospital-based care (desirable)- Understanding of the private healthcare sector (desirable)
