Person Centred Support Worker

**About Me**

I am a 50-year-old woman of Greek heritage living in Glandore. I have a hearing impairment meaning I do not hear properly; however, I am quite good at lips reading.

My preferred form of communication is AUSLAN (Australian sign language) but I also communicate via pen and paper. I would prefer my support staff to be fluent in AUSLAN.

I love reading and going to the library. I also enjoy shopping and going to hungry jacks.

**The Role**

An Opportunity is available to join my team of wonderful support workers who help to facilitate my daily life. My support workers assist me on a 1 to 1 throughout the day.

I struggle to manage my money and need my staff to keep an eye on my spending as sometimes I can't pay for the items I want which upsets me. I am also very protective of my money.

I don't like engaging with staff when it's time to take my medication and I don't have a strong insight into my physical and mental health needs. This means I sometimes have to go to hospital.

When I am frustrated, I will often turn my stereo up all the way to feel the vibrations.

**Key responsibilities**:

- Assist me with ADL's
- Personal care
- Manual Handling
- In home activities such as cooking and arts
- Support me to achieve my goals to reach my potential in all facets of my life.
- Support my life skill development through proper implementation of established plans and programs to support the achievement of goal outcomes for me.
- Actively engage in person centred active support.
- Maintain accurate records.

**Shifts**
I am looking for someone to support me on a casual/PT basis with the potential to join the team part time in the future.

My shifts are generally but not limited to 7:00 am to 3:00 pm, 3:00 pm to 11:00 pm and sleepover from 11: 00 pm to 7:00 am on the following day.
- NDIS Worker Screening or DHS Disability related check
- Current Driver's Licence
- Current First Aid Certificate
- Current CPR
- Certificate III in Individual Support (Disability/Aged Care) or other relevant qualification
- Diabetes Management
- Dysphagia training
- Manual Handling Level 2

**About you**:
I am looking for a creative and positive person to join my team. I am looking for someone who would help with my needs and goals.

I need support to learn how to plan, shop for, and cook healthy meals to help me manage my weight and diabetes.

I suffer incontinence, especially when I don't take my medication and require prompting to shower and change my clothes when this happens.

**Other requirements**:

- Experience in working with people who have experience with high behaviours of concern's
- Ability to implement individualised developmental programs.
- Experience implementing positive behaviour support strategies.
- Flexible working hours
- Experience in various models of support preferred.
- Ability to adapt quickly to changes in service delivery.
