**Your new team**:
CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. We are seeking applicants for a secondment cover for a Case Manager. This role is a unique opportunity to have a real impact in meeting the needs of one of our community’s most complex and vulnerable cohorts. You will form solid working-relationships within the pre-existing community teams, and foster strong therapeutic relationships with your clients. We support rehabilitation goals through the use of best evidence-based frameworks and assertive case management.

**Your contribution**:

- ** You are a team player with a commitment to quality client care**:

- ** Relevant professional qualification in a health-related discipline (nursing, social work, psychology or occupational therapy) and current registration with the relevant professional board, Association or College.**:

- ** Social Workers: Eligibility for membership with Australian Association of Social Workers (AASW)**:

- ** Minimum 3 years’ experience in a relevant mental health setting (including acute psychiatry settings)**:

- ** Demonstrated knowledge of recovery and collaborative clinical practice.**:

- ** A working knowledge of the Mental Health Act (2014), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines**
