**About your new role**:
You will be working closely with the multidisciplinary team and other stakeholders within the 68 bed facility. This position is responsible for the provision of person-centered, individual care to residents. Working as part of our dedicated team means you will have the unique opportunity to contribute to our residents pursuing their unique personal goals and passions, enabling them to live full and meaningful lives.

**Duties**:

- Promote the physical and emotional wellbeing of residents
- Ensure person-centred support and support plans are at the forefront of everything you do
- Partnering with residents to be involved in the meaningful activities of their life
- Coordinate with other staff to achieve outcomes and liaise directly with nursing staff regarding any specific health concerns
- Support with everyday activities such as;
- Eating
- Personal hygiene tasks (such as bathing, dressing, personal care)
- Mobility
- Household tasks
- Leisure activities

**Skills and Experience**:

- Ability to engage residents though a person-centered approach to care and support
- Works well and contributes effectively within a team environment.
- Demonstrated effective interpersonal and communication skills (written and verbal)
- Hold a Certificate III/working towards attaining one (desirable) or willingness to obtain one
