**About the Role**

We are seeking a **female Support Worker** to provide one-on-one assistance to an NDIS participant in **Oakleigh South**. The role involves a combination of **personal care** and **community participation** supports, helping the participant maintain daily routines and actively engage in the community.

**Key Responsibilities**
- Provide respectful and professional **personal care assistance**, including hygiene, grooming, and daily living support.
- Support the participant to access the **community**, attend activities, appointments, and social outings.
- Encourage skill development, independence, and confidence.
- Maintain accurate and timely **progress notes and documentation**.
- Build positive, trusting relationships with the participant and their family.
- Follow Breezy Care’s policies, procedures, and NDIS Practice Standards.

**About You**
- Female support worker (participant preference).
- Compassionate, reliable, and committed to providing person-centred support.
- Experience in **personal care** and **community access** (preferred).
- Excellent communication and interpersonal skills.
- Current **NDIS Worker Screening Check**, **Working With Children Check**, and **First Aid/CPR** certificate.
- Valid **driver’s licence** and reliable vehicle (preferred).
