**T**his is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.**

We have an exciting position that provides the opportunity to make a difference to young people and their families and carers and learn from a skilled, supportive, multidisciplinary team. The Social Worker - Youth Health will work within the Youth Health team to:

- provide intake and referral, holistic psychosocial assessments and brief interventions,
- actively support and guide young people with multiple vulnerabilities and/or chronic and complex healthcare needs to navigate and engage with appropriate support services,
- provide outreach activities to engage young people most at need, including in schools and in health facilities,maintain a strong working knowledge of key support services for young people on the Central Coast

This is an outstanding opportunity for someone who enjoys a diverse role. You will be well supported within a multidisciplinary team navigating and supporting vulnerable young people to engage with specialist and mainstream health care providers. You are passionate about client centred care and have a solid understanding of key support services for young people, particularly those with multiple/complex needs, in the local area.

Youth Health delivers holistic health care that is responsive to the needs of young people with multiple vulnerabilities, care that is coordinated and integrated with the range of health service providers and other government and non-government youth service providers. You will be working in a multidisciplinary team with medical, nursing, allied health and admin staff. Our service priorities are aligned with the NSW Youth Health Framework 2017-24, the District and Directorate annual priorities.

**About You**
- You have a Degree in Social Work from an accredited course, providing eligibility for membership of the Australian Association of Social Workers (AASW). Overseas qualifications must be accompanied by the AASW Overseas Assessment number.
- You can work collaboratively within an interdisciplinary team to deliver optimal care.
- You have strong verbal, written and interpersonal communication skills including the ability to adapt communication style to suit different audiences.
