**JOB SUMMARY & DESCRIPTION**
This role is for a Mental Health Support Worker at Conscious Care Support, a growing, and unique mental health service provider. As a Mental Health Support Worker, you will support individuals living with a chronic psychosocial disability. You will provide ongoing psychosocial, emotional, and practical support to help these individuals identify, work toward, and reach their goals.
This is an outreach role. This means that you will independently support individuals in their homes as well as in the community. You will also work collaboratively with the care team of an individual to ensure the best quality of support.

**QUALIFICATIONS & SKILLS**
- You will need to own a comprehensively insured car and have a driver's license.
- You will need to have a minimum of a Certificate IV in Mental Health.
- You will need to have the ability to write case notes and do this articulately and concisely.
- You will need to have a nuanced and detailed understanding of mental health.
- Above everything else, you will need to have genuine care and compassion to want to help others.
- We are wanting to find individuals who want to stay with us for the long term and play an instrumental role in our expansion and vision.

**WHY WORK AT CONSCIOUS CARE SUPPORT?**
Conscious Care Support is not structured like other community mental health providers. Whilst we have over 90 employees and service close to 500 individuals each week in the community, everybody at Conscious Care Support provides support work.

At Conscious Care Support, we ensure that all of our mental health support workers are guaranteed consistent, ongoing hours, and an opportunity to autonomously shape and manage a caseload that works in accordance with your lifestyle.
We are looking for individuals with at least 3-4 days per week of capacity (weekdays). **Specifically, you must have the following days/times available**:
**-Wednesdays (all day, 9am-5pm)**
**-Thursdays (all day, 9am-5pm)**

**Job Type**: Casual

Pay: $40.46 - $72.83 per hour
