**About the Role**

Caboolture Private Hospital is seeking a skilled Discharge Planner to join our team, working closely with the Medical/Surgical Ward Nurse Unit Manager (NUM) and their team. This vital role involves coordinating and facilitating smooth patient discharges, ensuring that patients receive appropriate follow-up care and resources.

**About You**

**As a Discharge Planner, you will**:

- Collaborate with medical and nursing staff to develop comprehensive discharge plans.
- Ensure timely, safe, and efficient patient discharges.
- Communicate effectively with patients, families, and healthcare providers to arrange follow-up care and services.

Caboolture Private Hospital is a fully accredited surgical and medical hospital situated in the outer Northern reaches of Brisbane. This hospital has 43 beds, 3 fully equipped operating theatres, a 12 chair Day Procedure Unit and a comfortable day oncology unit.
