Specifically, we’re looking for applicants of Aboriginal and/or Torres Strait Islander decent (An applicant's race is a genuine occupational qualification and is authorised under Section 14 of the Anti-Discrimination Act 1997 NSW). Someone who possesses Certificate III in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent qualification.

Here at Justice Health and Forensic Mental Health Network (the Network), the Aboriginal Health Worker will work within the Integrated Care Service (ICS) based in the health centres to increase access to health treatment and prevention services for Aboriginal people, and have a strong focus on the prevention and management of chronic disease with an emphasis on improving the patient journey from custody to community. The Integrated Care Service aims to support and enhance the existing systems to provide an integrated model of care for adult patients in custody and on release into the community with chronic or complex health conditions. Furthermore, ICS is to monitor patients from entry at reception through custody and post release unto the community, ensuring they receive seamless, timely, appropriate and effective health care. The role and responsibilities are to be carried out in a manner that is consistent with Network delegations, policies, procedures and operations systems, and in line with both the NSW Health and Network Codes of Conduct.

- Possess a current NSW drivers licence and the willingness to travel across the Network (as necessary).
- Possess the ability to work independently and as part of a team.
- Recent experience working with Aboriginal and/or Torres Strait Islander patients and their families and knowledge of their local cultural-specific communities and its health services.
- Demonstrated understanding and commitment to the values of the Justice Health and Forensic Mental Health Network.
