**Role Summary**:
Our Mental Health Community Workers are experienced Community Workers whose roles have an emphasis on working with clients with psychosocial support requirements. The role works within our Mainstream Capacity Building (MCB) team and aims to reduce the readmission cycle to hospital for clients, as well as focusing on barrier minimisation strategies for people living with Disability.
**Responsibilities and Duties**:
- Assisting clients to live independently and to access supports and services
- Developing support plans based on the needs of individual clients
- Monitoring the progress and providing reporting on support plans
- Liaising with the families and representatives of clients
- Seeking feedback and ensuring this is used to improve services
- Networking and connection to services
**Skills, Attributes and Experience**:
**As well as the above, the Key Requirements include**:
- Applications must possess a Cert IV or higher qualification in a relevant field
- Possess or be willing to obtain a NDIS Worker Screening Check
- Hold a valid driver’s licence and a reliable vehicle
- Previous experience working within mental health is highly desirable
