**Position Overview**:
The Welfare Social Worker is responsible for delivering professional and compassionate support services to individuals and families in need. This role involves conducting assessments, developing care plans, and advocating for clients to access appropriate services and resources. The Welfare Social Worker will work collaboratively with community partners, allied professionals, and support networks to address the social, emotional, and financial needs of clients.

**Key Responsibilities**:

- **Client Assessments**:

- Conduct thorough needs assessments to understand the social, emotional, and economic factors impacting clients’ well-being.
- Identify clients’ strengths, challenges, and areas where intervention is required.
- **Case Management**:

- Develop and implement individualized care plans aimed at addressing the unique needs of each client.
- Provide ongoing case management, including monitoring progress, reassessing needs, and adjusting care plans as required.
- Coordinate and refer clients to relevant services such as housing, healthcare, legal aid, financial assistance, mental health support, and community resources.
- **Advocacy & Empowerment**:

- Advocate on behalf of clients to access services, benefits, and resources.
- Empower clients to make informed decisions about their welfare, providing guidance and support as they navigate complex systems (e.g., social services, healthcare, legal systems).
- Educate clients on their rights and responsibilities, ensuring they have a voice in their care decisions.
- **Crisis Intervention**:

- Respond to urgent and emergency situations, providing immediate support to individuals with disibilities in crisis (e.g., homelessness, domestic violence, mental health emergencies).
- Develop safety plans and connect clients to emergency services or shelter when necessary.
- **Collaboration & Networking**:

- Build and maintain strong working relationships with local agencies, community organizations, and service providers.
- Participate in case conferences and multidisciplinary team meetings to ensure comprehensive support for clients.
- Work collaboratively with other social workers, psychologists, health professionals, and legal advocates to provide holistic care.
- **Documentation & Reporting**:

- Maintain accurate, timely, and confidential records of all client interactions, assessments, and care plans.
- Prepare regular reports for management and ensure compliance with organizational and legal requirements.

Pay: $80,000.00 - $100,000.00 per year

**Education**:

- Bachelor Degree (preferred)

Licence/Certification:
