**About the Role**:
As a Community Worker with Support Foundation, you'll be supporting individuals with disability, helping them navigate day-to-day challenges, connect with services, and achieve their goals under the NDIS. Every day will be meaningful, hands-on, and full of opportunities to create impact.

**What We’re Looking For**:

- A tertiary qualification in **Community Health, Social Work, Disability Services** or similar
- A valid **NSW unrestricted driver’s licence**:

- **Fluent English** speaker - with a **second language** being a bonus
- Completed all **NDIS Worker Orientation Modules**:

- **NDIS Worker Screening Check** completed
- A **genuine interest** in supporting people with disability
- A **can-do attitude** and willingness to grow with a passionate team
- **NDIS sector experience** is desirable, but not essential - we value motivation and heart just as much

**Role Overview**

This role involves working closely with people with disability and their families to help them live independently, achieve their goals, and access the right services and supports under the NDIS. You will provide ongoing support, guidance, and coordination, helping clients navigate daily challenges and connect with community resources.

**Key Responsibilities**:

- **Understanding Client Needs**:
Talk with clients and their families to understand their personal needs, challenges, and goals.
- **Planning and Coordinating Supports**:
Help create support plans that include services like personal care, therapy, transport, employment, or social activities.
- **Monitoring Progress**:
Regularly check in with clients to see how things are going, keep records, and adjust plans if needed.
- **Referring to Other Services**:
Link clients to other providers, community groups, or services (like housing, job training, or mental health support) if extra help is needed.
- **Connecting with the Community**:
Work with local community groups, service providers, and government agencies to make sure clients have access to all available supports.
- **Supporting Daily Living**:
Hands on support. Help with everyday tasks like personal care, meal preparation, shopping, social participation, and attending appointments.
- **Educating Families and Carers**:
Guide and support families or carers so they feel confident in helping their loved ones live safely and independently.
- **Support in Justice or Youth Services (if applicable)**:
Work with people under court orders or young people who are struggling, helping them re-engage with school, work, or community life.
- **Funding and Reporting**:
Help prepare reports and funding requests to NDIS, government bodies, or other agencies as part of maintaining support.

**Job Types**: Full-time, Part-time

Pay: $23.23 - $47.06 per hour

Expected hours: 20 - 40 per week
