- Employment Term: 3 Fulltime opportunity offering Monday to Friday working hours
- Location: Redlands, Logan River Valley & Brisbane South
- Remuneration: $44.92 - $46.95 p/hr (additional 25% loading for casual hours) + 10.5% Super + Not for Profit Salary Packaging + Penalties & Allowances

**Your part in our BlueCare team**
- As a Home Care Package Partner, you’ll partner with clients and their families to ensure that their needs are met through providing information, advice and support. You will identify their personal goals and aspirations to determine the most effective way to support and achieve them- You will support our clients in developing their personal support, budget plans and coordinate & monitor the client support services. This includes all case management and procurement activities for our clients.**What makes a difference for us?**:

- Experience in coordinating or assisting in the care coordination of a community-based service or client care
- Knowledge and understanding of Consumer Directed Care facilitation and relevant legislative frameworks, standards and funding requirements
- Previous experience of budget processes at a client level and case management/coordination
- A current driver’s license and roadworthy registered vehicle
- Certificate III in Community/Aged Care (additional qualifications will be highly regarded)
