
Service Coordinator

At **Safe Place for All** we are currently recruiting for a part time Service Coordinator who will play a crucial role in liaising with staff, intake of new NDIS participants and their families and ensuring that our services are exceptional and delivered appropriately. You will also be responsible for answering incoming calls from clients and employees promptly and courteously, providing clear and accurate information as well as rostering to ensure all clients are seen by our Support Worker.
Three full days (Flexibility with regard to specific days Monday - Friday will be considered).
**Role Specific Requirements**:
As a Service Coordinator your main duties will include Coordinating and overseeing the delivery of services to our participants, ensuring their needs and goals are met effectively. Health related goals will be a primary focus of this role.
- Collaborating with participants, their families, support teams and allied health professionals to develop individualized care plans that align with NDIS goals and guidelines.
- Lead a team of staff, who primarily work remotely in the community, providing ‘drop in’ and Community participation services to people we support living in their own homes.
- Conducting comprehensive assessments of participants' clinical needs and developing appropriate service plans accordingly.
- Liaising with healthcare professionals, therapists, and service providers to ensure seamless coordination and integration of clinical services.
- Monitoring the quality and effectiveness of clinical interventions and adjusting as necessary to optimize outcomes for participants.
- Providing clinical guidance, supervision, and training to support staff to ensure a high standard of care is maintained.
- Assisting with the development and implementation of policies, procedures, and best practices related to clinical service delivery in compliance with NDIS regulations.
- Collaborating with other stakeholders, such as NDIS coordinators, allied health professionals, and community organizations, to enhance participant outcomes and promote a multidisciplinary approach to care.
- Maintaining accurate and up-to-date documentation, including progress notes, incident reports, and service records, in accordance with NDIS requirements.
- Keeping abreast of changes in NDIS policies, guidelines, and best practices related to clinical services and ensuring compliance with all relevant standards.
- Collaborate with organizations to host events and workshops that promote dialogue and learning related to disabilities.
- Engage with external disability service providers, community organizations, and agencies to expand support options and stay informed about available resources.
- Represent the SPFA service to government, community agencies and the wider community.
- Actively participate in conferences, seminars, and professional development opportunities to remain up to date with industry trends and advancements.
**Qualification and Experience**:
**Essentials**
- High knowledge and understanding of the National Disability Insurance Scheme (NDIS)
- Awareness of disability sector and people with disabilities.
- Tertiary Qualifications in Disability support, Social Work, Community Services or related fields.
- Current Valid Driver's License.
- Ability to work independently and as a team member.
- High level of verbal and written communication skills
- High level of computer literacy with competency in Microsoft Office programs including Word, Excel, Publisher, Outlook
- Ability and willingness to work Monday
- Friday.
- Prepared to participate in on call roster within Service Coordinators team after hours and over the weekends/public Holidays.
