With our prosperous participant base and growing team, we are on the search for committed, professional, and compassionate individuals to join our team with the opportunity to have a career in managerial roles and full-time positions.

We are currently looking to hire a **Support Coordinator** who has experience working within the NDIS and supporting people with physical, intellectual, and psychosocial disabilities ensuring that they are implementing their NDIS plans, building their capacity, and achieving their goals.

- You MUST have a current driver's license and preferably own car to use on shifts
- You MUST be fully vaccinated for Covid-19
- Company Laptop and Phone provided

**Your Key Responsibilities Include (Not Limited to)**:

- Build rapport and collaborate effectively with staff/management, participants, their family, and friends to achieve a positive outcome and ensure a smooth service delivery flow throughout the participants' plan.
- Assist with liaising with participants' support networks including allied health, families, and friends to explore new goals, ambitions, promote and empower participants to achieve their goals and provide a medium of solid communication.
- Flexible and adaptable
- Understanding mental health and how it can impact individuals’ wellbeing
- High attention to detail
- Advanced skills in writing/reading observations and assessments
- Strong ability to multitask and build rapport with individuals from different background

**Essential Requirements**:

- A Qualification in Mental Health, Community Services or Disability Services or related industry.
- 12 months minimum experience in providing support coordination services, or similar role, support people with physical, intellectual, or psychosocial disabilities
- COVID Vaccinations
- Willingness to travel
- A safe, reliable vehicle with comprehensive car insurance
- WWCC or willingness to obtain
- Police Check or willingness to obtain
- NDIS Worker Orientation or willingness to obtain

As a support coordinator in our team, you will be responsible for supporting individuals and ensuring that they have a quality of life. The role will involve working with participants with both low and complex needs.

**Salary**: $75,000.00 - $85,000.00 per year

Schedule:

- 8 hour shift
- Monday to Friday
