**About the Role**

**Young Breeze** is seeking a caring and reliable **Disability Support Worker** to join our team, supporting clients in **Caulfield VIC**.
You will help participants achieve their NDIS goals, promote independence, and enhance their overall wellbeing.

**Working Hours**:

- **Monday**: 9:00 AM - 1:00 PM
- **Tuesday**: 9:30 AM - 1:30 PM
- **Friday**: 10:00 AM - 2:00 PM

Additional shifts may be offered based on client needs.

**Key Responsibilities**
- Provide personal care and daily living assistance in home and community settings
- Support clients with meal preparation, shopping, and light housekeeping
- Encourage skill development and community participation
- Record accurate progress notes and follow individual care plans
- Maintain safe, respectful, and inclusive work practices

**About You**
- **Certificate III in Individual Support (Aged Care/Disability)** - _essential_
- Current **Police Check**, **Working with Children Check**, **First Aid & CPR**, and **Driver Licence**:

- Compassionate and professional approach
- Excellent communication and interpersonal skills
- Reliable, punctual, and proactive
- Previous experience supporting NDIS participants is an advantage
