We are seeking a Personal Care Assistant to join our dedicated team at Healthcare at Home. This position will work across the Perth metropolitan area.

Under the direction of the Nurse Manager/Registered Nurse, the Personal Care Assistant will provide personal care requirements to clients in their place of residence in the community and is responsible to the Nurse Manager/Registered Nurse for standards of practice in relation to the provision of care including all infection control issues.

The Position- Under the direction of the Nurse Manager/Registered Nurse and the Care Plan, perform the personal care tasks as required for the client.- Provide a range of personal care tasks based on assessed needs as determined by the care plan, which enables the recipients to maintain their desired standards of personal care and hygiene including:
- Assist bathing, sponging or showering,- Assist with toileting,- Assist with mobility such as getting out of bed, chair, commode or wheelchair,- Assist clients with self-management of medication.- Encourage and promote recipients independence and coping mechanisms.- Assist with medications as per scope of practice.-
- Documentation completed and recorded in client electronic record after each visit.

You will have experience working in a community, palliative or dementia setting, high level of interpersonal and communication skills and able to work effectively in a multi-disciplinary team environment.

Successful applicants will hold a Certificate 3 in Home & Community Care, Aged care, Disability or Individual Support, a current WA drivers’ licence and poses a valid First Aid & CPR certificate.
