Position Description

Position Purpose: Home Care Workers (HCW) provide high-quality care and support to clients in both home and community settings. This role involves delivering a range of clinically-based and personal care services to people of all ages with varying levels of needs, complexity, and intensity. The HCW works under the supervision of their Team Leader and supports the goals and objectives established by treatment plans and support plans issued by Registered Nurses (RN) and Enrolled Nurses (EN) on OSAN’s Clinical Care Team.

Main Tasks: may include any of the following on an ongoing or one-off basis and are based on what qualifications you hold.
- Social Needs
- Care Coordination Support: Liaising with allied health professionals to ensure coordinated care.
- Carer Training Assistance: Providing training and support to carers to enhance their caregiving skills.
- Emotional Support Assistance: Providing emotional support and companionship to clients with mental health conditions.
- Physical Needs
- Fall Prevention Assistance: Implementing fall prevention strategies and assisting with mobility and transfers.
- Continence Support: Assisting with continence management, including the use of incontinence aids and toileting schedules.
- Incontinence Aid Assistance: Assisting clients with the use of incontinence aids.
- Toileting Schedule Support: Developing and implementing toileting schedules to manage clients’ needs.
- Nutrition and Hydration Support: Assisting with meal planning, preparation, and ensuring adequate hydration.
- Health/ Medical Needs
- Vital Signs Check: Regular monitoring and recording of blood pressure, blood sugar levels, and other vital signs.
- Medication Prompting: Reminding and assisting clients with medication adherence as per the prescribed treatment plan.
- Pain Relief Assistance: Assisting with pain management strategies, including medication prompting and physical comfort measures.
- Wound Care Support: Monitoring around wound dressing for any signs of infection or changes of skin condition. Does not include dressing changes.
- Catheter/Stoma Maintenance Support: Supporting the care and maintenance of catheters and stomas.
- Psychological Needs
- Cognitive Support Assistance: Supporting clients through cognitive therapies and daily activity assistance.
- Emotional Support Assistance: Providing emotional support and companionship to clients with mental health conditions.
- Complex Needs
- Respiratory Support Assistance: Assisting with respiratory care, including the use of nebulizers and inhalers.
- Comfort Care Support: Providing palliative care support to enhance comfort and quality of life for clients with serious illnesses.
- Health Education Reinforcement: Reinforcing education provided by RNs and ENs on managing specific health conditions.

Other Duties
- Maintaining accurate and up-to-date client records.
- Participating in team meetings and case conferences.
- Adhering to workplace health and safety policies and procedures.
- Engaging in continuous professional development and training.

Person Specification

Qualifications may include any of the following and determine which tasks you are authorised to deliver.
- Certificate III in Individual Support
- National Police Check
- NDIS Worker Check (NDISWC)
- Current First Aid and CPR certification
- Working with Children Check
- Diploma of Nursing, Social Work or Community Services (or studying towards one)
- Mental Health First Aid
- Manual Handling
- Medication Assistance Certificate

Experience
- Previous experience in a similar role within the healthcare sector.
- Experience working with clients with complex care needs.

Skills & Competencies
- Competency in performing assigned clinically-based tasks.
- Ability to demonstrate dignity and respect when delivering personal care and other support services.
- Strong problem-solving skills and ability to respond to emergencies.
- Excellent team player, proactive communicator with attention to detail.

Pay: $38.00 - $48.00 per hour
