**About the role**:
This role at its core is about developing strong connections with clients ensuring they receive the support they need to continue living the life they choose at home and in their local community.
- Building relationships based on trust with prospective and existing clients and their families
- Meet with prospective clients and their families to understand their goals and current needs and provide solutions
- Conduct regular quality assurance visits with clients in person and/or over the phone to ensure the highest level of care is being attained
- Be knowledgeable about a wide range of resources and local services
- Build relationships and communicate regularly with other health service providers and/or local referral sources

- Enrolled Nurses and Registered Nurses looking for a change are encouraged to apply
- Experience in case management, assessments, care planning
- Reliable, adaptable, motivated and committed to providing quality care
- A current Working With Children’s & Vulnerable People card
- Current and required flu and COVID-19 Vaccinations
- Have Australian working rights and are able to pass a national police clearance
