A genuine interest in the wellbeing & inclusion of people with disabilities. **Licence/Certification**: - **Certificate IV in disability (required)**: - NDIS Worker Orientation Inductioon Modules (required) - First Aid & CPR (required) - NDIS Worker Screen check (required) - Australian National Police Check (required) - Full drivers license (required) - Working with Children Check - Employer (required) - Infection Control Training - COVID 19 (required) - Fully vaccinated (three doses which includes the booster dose) - mandatory **Essential Criteria** - Passionate about caring and committed to client wellbeing - Previous experience supporting people with a disability - **A reliable vehicle with full comprehensive car insurance.** **Qualifications & Experiences** - **The role will involve community access with activities such as swimming, shopping and dining out. You will be comfortable with Swimming, as you will be required to enter the pool and assist the client "**amputee"**: - Tuesday, Wednesday, Thursday, Friday and Saturday**: - Support clients to participate in age-appropriate activities and tasks including employment, in home support and community access. - Support clients to access activities and outings in the community. - Provide direct assistance to each client in accordance with their service plan and as directed - Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality. - Monitor the safety and well-being of the clients and report any concerns to the Manager. - Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required. - Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes. - Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals. - In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals. - Ensure that all programming undertaken is appropriately maintained, documented and reviewed. - Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress. - Liaise extensively with other staff to ensure the overall smooth and consistent running of the service. - Report regularly to manager - Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review. - Provide backup and support to other staff, as necessary. - Participate in Performance Development meetings to identify ongoing training and support opportunities. - Notify the service if you are unable to work your rostered shifts, with as much notice as possible. - Contribute to Case reviews with Employment Consultants and Facilitators. - Take reasonable care for his or her own health and safety - Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons. - Report any quality and safety incident or hazards at work to their Manager or Supervisor. - Carry out their roles and responsibilities as detailed in the relevant policies and processes. **Job Type**: Casual **Salary**: $30.00 - $33.00 per hour **Benefits**: - Employee mentoring program Schedule: - On call COVID-19 considerations: *Please Note: To ensure the health and safety of our clients/participants, the people we serve at EddenRose Community Services, all our workers are required to comply with statutory and Government requirements, such as public health directions etc. **Education**: - Certificate I - IV (required) **Experience**: - disabilities: 1 year (required) **Language**: - English (required) Licence/Certification: - Full Driver Licence (required) - Working with Children Check (required) - First Aid Certification (required) - police check (required) - NDIS Worker Ori
