**About the Position**

**We are looking to welcome a** Family Support Practitioner **to deliver an outreach model of family case-management programs to families.**

- **Full time position.****:

- **Role is government funded until 30 June 2027 with possible extension.****:

- **Level 5 under the SCHCADS Award. Salary range is $85,127 - $91,588 + Super.***

**_Key Responsibilities:_**

- **Maintain a caseload and regularly attend and participate in case reviews.****:

- **Engage families and develop case plans addressing unmet needs and child protection concerns.****:

- **Work collaboratively with other professionals and agencies to ensure families goals are achieved.**

**About You**

***

***:

- **A tertiary degree in a relevant discipline (allied health, social work, human services).****:

- **At least 2 years’ experience providing case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework (preferable).****:

- **Cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds, including Aboriginal and Torres Strait Islander people.***

- **Current driver’s licence.****:
